| Synopsis: | The M/V "Alexia", a bulk carrier sailed from Dublin port on the night of the 4th of February 2004. Whilst disembarking the pilot in Dublin bay, the pilot ladder side ropes gave way. The pilot and approx 27 feet of pilot ladder fell into the pilot cutter below. The pilot was seriously injured. |
